We are so thankful for the gifts of your time and talents! Because the safety of everyone in camp is our first priority, all staff are required to complete the four components of our screening process listed below.

1. Submit a completed Camp Staff, Returning Camp Staff, or Junior Counselor Application (staff over 18 years of age who have served within the past 15 months in NEA/OLA/SEGA Camp Ministries may submit a Returning Staff application);
2. Submit 2 personal references, returned to address on form by referring individuals (required in first year);
3. NEW PROCESS: Complete the online Missouri Family Care Safety Registry (FCSR) Worker Registration Form (staff over 18 years of age), a one-time registration for anyone not already registered with the FCSR.
Instructions: https://webapp01.dhss.mo.gov/FCSRWebRegistration/Welcome.aspx. Cost is $10 plus a $1 processing charge, which must be paid with a credit card. If for any reason you are unable to complete the process online, please contact Susan Moore at smoore@eden.edu or by phone at (314) 918-2515 (first year).
4. Submit a completion certificate for the online Boy Scouts of America (BSA) Youth Protection Training (good for 2 years).
Links to downloadable forms for items 1-2 are below. Since all forms must be signed, they need to be either mailed to the Northeast Area Office at P.O. Box 10230, Columbia, MO 65205 or faxed to 573-442-1925.

Please also complete the Staff Brief Health History (form below) and bring it with you to camp.
